At Parkallen Dental, located in Edmonton, Alberta, we are committed to protecting the privacy and confidentiality of your personal and health-care information. We adhere to the requirements of the Health Information Act (HIA) of Alberta, the Personal Information Protection and
Electronic Documents Act (PIPEDA) and other applicable privacy laws.

We have adopted this Privacy Policy (“Policy”) to inform you about how we collect, use,
disclose, safeguard, retain and destroy your information, and about your rights in relation to that information.

2. What information we collect
We collect two main types of information:

• Personal Identification / Contact Information: such as your name, address, phone
  number, email address, date of birth, and emergency contact.
  
• Health & Dental Information (“Health Information”): such as your medical/dental
  history, treatment records, diagnostic images, insurance information, billing records, appointment history and related clinical information.
  This is consistent with the guidance that dentists (as custodians) must collect and manage
  health information in accordance with HIA standards.

We collect information in various ways: when you complete registration forms, provide clinical information, visit our website or contact us, request appointments, fill out insurance information, or otherwise communicate with us.

We may also collect information directly from you, your legal guardian (if applicable), or via
third-party providers (e.g., laboratories, imaging centres) with your consent.

4. Why we collect, use and disclose your information
We will collect, use and disclose your information for legitimate purposes, including:

• Providing dental and oral-health care and treatment.
• Managing and operating our dental practice, including scheduling, billing, insurance
  claims and follow-up.
